A rotator cuff injury affects the tendons of the four muscles that surround and control the shoulder (most often the supraspinatus tendon) — not just the muscle itself. It is common from about 20 to 50 years of age, often from gym and sports activity, and can make certain arm movements difficult.
Left unaddressed at a younger age, it can cause problems later.

How it feels
Pain is often present only with particular movements rather than all the time — for example lifting the arm overhead, reaching behind, or taking something down from a height. It is typically a dull ache felt deep in the shoulder, and pressing on the area can bring it out.
First steps after injury
For a fresh injury, a common early approach is rest, ice, compression, and elevation. Ice can be applied for about 10 to 20 minutes, up to three times a day, for around three days.
A mild injury often settles in that time. If pain continues beyond three days, it is worth having it assessed.
How physiotherapy helps
Assessment identifies which structure is involved and whether there is a tear, and treatment first targets the pain. To support healing and a safe return to sport and gym activity, exercises are introduced and customised to your muscle strength.
Returning to sport before the shoulder has been strengthened carries a real risk of further injury, so warm-up and cool-down are important, and heavy lifting and throwing with the affected arm are best avoided until recovery is further along. If movement becomes very restricted, it is worth checking for frozen shoulder as well.
