Sacroiliac (SI) joint pain is often reported as low back pain, but on assessment it is usually felt slightly to one side of the spine rather than in the centre. The sacroiliac joint is where the sacrum, at the base of the spine, meets the pelvis.
Recognising it matters, because it can be mistaken for a disc problem or a hip problem.

How it presents
- Pain to one side of the spine, sometimes tender to touch
- Pain when standing up from sitting, or when climbing stairs, usually on one side
- If left unaddressed, it can radiate into the leg or hip
Because it can refer into the leg, it is sometimes confused with a disc problem or a hip joint problem.
What causes it
- Muscle or ligament injury around the joint
- A fall, particularly landing to one side, which can jolt the joint
- Sitting with weight loaded onto one side, or lifting with poor mechanics, which can set up weakness over time
After a fall, pain may be severe at first and then ease. If a low-grade ache continues for several days to a week, it is worth having it assessed.
How physiotherapy helps
Assessment establishes whether the pain is coming from the SI joint or the back, and an X-ray or MRI may be suggested if needed. Where a muscle or ligament injury is involved, treatment first helps it settle and reduces swelling and pain.
To reduce recurrence, strengthening and stretching exercises are taught for the muscles that had become weak, customised to each person. If symptoms radiate down the leg, related conditions such as sciatica may also be considered.
